I issued a refund on an item that wasn't delivered and then later it was finally delivered.  Is there a way I can open the dispute back up to get paid for the item that was finally delivered?

No.  What you can do is use the CONTACT BUYER link and let the buyer know that you see the package has finally arrived, and you would like to be reimbursed $x.xx .  Include your Paypal email address, and hopefully the buyer is honest and will reimburse you.
