How to file a damage claim with UPS

How do I file a damage claim for UPS for an item that I sold that was damaged in transit.

If you bought the label here on eBay, go to this linked page and scroll down to the section on UPS claims.

There is a specific form and email you must use to file the claim with UPS.

Hi,

Thanks for directing me there.  I started to fill out the form but was stumped on "consignee's email & phone number."  Is the consignee the buyer and if so, how do I go about getting their email and phone number?

Both are in the Order Details - their email is an eBay "alias" email, that only shows there for 2 weeks I think.
