How do you change your feedback?

I had left negative feedback for a seller who  had ab item I bought who listed it twice and then canceled mine (I bought it first for a lesser price) and was selling it to the 2nd listing buyer (who bought it for more $$).  They would not respond to my messages.   I had also bought other items from them.  I left negative feedback on the canceled item.  I received my other items today and in the box was the canceled item... I feel bad now and want to change my feedback.   How do I do this???

The only way to change feedback is to ask the seller to send you a Feedback Revision Form.  This can only be done within 30 days of the feedback being left and the form must be acted on within 10 days.  The form can only be sent once and sellers receive a very limited number of these forms each year.
