I have not been able to complete a "refund request" form, without sending the items back.  The seller has agreed to send me a refund without returning the items... I do not know how to do this.

Did you open a case of return item from your purchase history?  This is what needs to be done.  One of the options for the seller is to refund without requiring a return.  This is something the seller has to do, not you.
