Creating Promotional Shipping Rules applied only to Domestic USA shipments

Looking for assistance as to how to create a promotional shipping rule that applies ONLY to USA shipments and is not applied to international shipments.

I messed with this a while back (set up a rule for "orders over $25 and shipping was included") and it seemed to work fine until one day I got an order from a buyer in the UK and it applied the rule to his order as he had ordered over $25.  Clearly, this is not meant to apply as the "free" shipping in this instance was more than the cost of the items.

I tried to ask customer service how to do this and they were absolutely useless.
