I wasn't able to find a way to do that.  Of course, I'm not a seller, so I don't have access to all the options that a seller has.  In case it turns out that what you want to do isn't possible, would you care to tell us why you want to do that?  Maybe there's another way to accomplish the same thing.

Only Alaska & Hawaii.  You can not block any of the lower 48.

No, not without blocking from the others.  The only carve-out of US states from the whole is for AK and HI (plus PR Guam and other non-state possessions).

And in order to block anyone based on address (and it goes by what the buyer has selected as his/her primary shipping address, not the registration address you see on his/her profile) you have to BOTH exclude that location (that is if it is already included in a broader location you do set up to ship to, such as US if you want to exclude AK/HI)  in the shipping setup for that listing AND set up a Buyer Requirement to block those with primary shipping address in a location you don't ship to.
