If a buyer purchases two or more of the same item and the seller is using calculated shipping there is a chance the rate will be way off due to dimensional shipping charges which calculates the shipping cost on the size of the package in lieu of weight. eBay shipping will have the correct weight on multiple items, but will keep the size of the package as if the buyer were purchasing just one. According to eBay the ONLY solution is to charge a FIXED shipping fee. This is a system problem that has been around eBay for quite some time. I've contacted eBay more than once about this, but it persists. Maybe with this post and others chiming in the issue can be resolved. I think calculated shipping is the fairest for buyers, but eBay leaves no choice since they do not automatically increase package size on multiple orders from the same listing.

I would not expect a solution to this any time soon. It is not as easy as adding dimensions together, since this would cause a problem for as many items as it would solve.
You can set a calculated shipping rule to offer no discount on combined items. I don't know if that would be better than using a flat rate with a separate cost for each additional.
