I sold an item and the tme I listed to ship the item was 2 days. The item has not been paid yet and the 2 days have passed. Do I have to ship the item before recieving payment?

No, you only ship when you receive CLEARED payment (if an echeck is issued, wait for it to clear)

The time for handling starts when the payment clears, not at the time of the transaction.  You wait until you see the payment in PayPal and PayPal says, "OK to ship."
