i bought a packaging slip through ebay and the dimensions on it say 4 x 4 x 7. tha'ts not what i entered, and when i googled apparently this is common. but the threads i found did not have any conclusion, like i sent it and it got sent back or i sent it and all worked out ok. anyone have any expierence with this?
also, my slip to italy has four pages. all have the same info on one has shipping bar code, one says customs decleration, one says dispatch note, and one says sellers copy. i figure i attach the one with the shiping bar code to the outside of the package and keep the other two, but what about customs decleration and dispatch notes?

The clear plastic self-adhesive Customs Form Envelope is Form 2976-E. If you need a lot of them, I think you can order them from usps.com. They're for Priority Mail International or higher. You put the first three pages of the 4-page form into that envelope. Leave it unsealed when handing it over at the post office as they like to red-datestamp the bottom right corner of each page before sealing them in. The fourth page is for your records.
First Class International packages use a one-piece label/Customs form that's around 4"x7" in size. You need only sign and date that in the lower left corner where indicated, then tape the label carefully to the box and send it out.

By "packaging slip" I assume you mean postage label.
Depending on the actual dimensions of the package, the label defaulting to 4x4x7 may or may not be a problem. If the package is significantly bigger it may make a big difference. If the package is near to that in size or smaller, it won't make any difference as long as the mailpiece qualifies as a package (and not a letter or large envelope).
The international label with 4 pages: you keep the Seller Copy. The other 3 copies go into a plastic sleeve that you can get from the post office, and this is attached to the package. You need all 3 of those with the package.

thanks! went to the post office and they showed me the way.
didn't worry about the incorrect package dimensions. have spent long enough trying to figure out one purchase.
sorry @muttlymob. i now see you got the answr in first. i'm just not careful enough about picking accepted solutions. hopefully we all don't care. i do appreciate all replies and the info.

for future sellers googling the 4 x 4 x 7 issue, it cleared (italian) customs no issues.
