I have shipped hundreds (maybe thousands) of these so far, and I finally had a customer say an item was damaged. They provided pictures of the damaged card. I contacted eBay phone support but didn't really get an answer. Basically, do I need to have the item shipped back to me for insurance to get paid out? I don't really want the card back since it is damaged, and it seems like a hassle to get the customer to send it back to me. Thanks for any help.

No. You can refund without requiring a return and use the buyer's photos and screenshots of the buyer's request to file your insurance claim.

Considering that $20 is the highest sell price using ESE and your cost should be a fraction of that you just refund and move on.

Why move on when there is insurance available via a simple online form.

How much was the price of the card? If you have sent thousands of these I would just refund and move on.
If the sale price was $12.00 or $15.00 I would file a claim to get your money back.

Because it is not worth your time nor the aggravation if your claim is rejected which is often the case.
