I had a buyer who returned an item claiming it was "not as described." I paid their return shipping and received it back without a box today. When I asked him about it he replied "the box was thrown away when opening." Is there anything that can be done? I'm not sure how this is even fair.

Nothing can be done besides to issue the refund, if it shows delivered to you. 'Fair' doesn't really apply. Hang in there...

so a buyer can put in writing that they sent an item back without all contents and that's fine? Interesting.

I don't see that you're a TRS (under this ID), and I can't see the listing... was it possibly sold under another ID?
There are deductions that can be made for refunds if you are a TRS offering 30-Day Returns (or better), or if you offered 'Free Returns'. Should one of those be your case, you can make a deduction relative to what the missing box did to the value of your item.
Note: I just read your last post... 'No Returns' gives you no leeway.

At least you got the item back. Ebay gave my customer the item and the money. Ebay is not for sellers.
