OK, so I am selling a rather expensive item to a new eBayer.
They are driving from London to Bristol to have a look at the item and pick it up.
So how do I prove they have picked it up and even better is there a way for them to say they have looked at the item and are happy with it.
Basically I am trying to avoid them saying they did not pick it up or if they return it they could just return a simelar sized weight 'empty' box and I am screwed.

I am usure why peope are saying that local pickup does not come with protection.The MBG applies to local pickup.As the blues have stated befor the MBG absoluately applies to a picked up item,
if you sell a buyer a 500 KG pool table and they want to return it the MBG still applies,you would still be on the hook for the return shipping ths is why you want to get cash
its very easy to come out a loser when you sell loacl picup unless you take cash

Insist on cash only, and you will not have any worries.

It seems like the OP is concerned about protection for him as a seller, not for protection for the buyer.

Again, local pickup items should ALWAYS be paid in cash when the buyer picks up the item.
No chance of a buyer opening an item not received case that way. Think about it.

Photo is always an excellent way of proving they have received the item, and take a hand written or typed receipt with you and get them to sign it before you give them the goods.

I do realize this but anyone that sells local pickup may not be aware that the MBG still applies to unhappy buyer who just hauled away a piano,when they file a SNAD you are on the hook for the return of a piano,not the buyer.
many people are just not aware that the buyer does not lose protection of the MBG on a local pickup
