So, say I sell something that is pricey enough (hope, hope) that it requires signature confirmation for delivery. Then say I get or accept a return request and send a label. Is the return label required to have signature confirmation on it?

If an ebay generated label, package tracking.
If you send a label, it's up to you

If the original shipment requires signature confirmation because the amount of the sale is $750+ then signature confirmation is required on the returned shipment as well. Even if it’s an eBay generated label.

I believe that "total cost" includes the cost of postage.
There is never any harm is buying Signature Confirmation. It costs $1.50 here from Canada Post, not sure what USPS charges.
But it puts the recipient on notice that you were not born on a turnip truck.

I believe it is $3.00 retail, $2.55 when added via ebay label and FREE when one buys (oh the HORROR!) insurance.
But then again, there are some savvy experts on USPS that can likely provide a direct link, which might show I'm all wet.
