That is something that each seller has to decide for themselves. There is no one size fits all answer.
Yes if you use Promoted Listings you have to pay more in FVFs to Ebay. How else would that work? If sellers didn't pay more for using Promoted listings, then everyone could use it for free. Then every listing would be Promoted which would render the option absolutely useless.
If you feel paying more in FVFs to promote your listings, then you should do that. The seller sets the additional FVFs they are volunteering to pay if the item sells from 2% to 100%. Ebay suggested rates are just that, suggestions, the seller decides how much more they are willing to pay for a sale.

Nothing is free. If you "promote listing" (EBAY will show your item to more buyers but there is no guarantee that you will get more sales).
You decide what percentage are willing to give to EBAY (higher percentage...better coverage). If customer clicks on "promoted listing" when your item sells you pay "regular selling fees + extra promoted fees".
If your items are "overpriced" customer will not make the purchase. If you click on "your listing" you can see what your competitors are charging for the same item.

Yes you have to pay.
It works for some people, it does not work for others.
Obviously, it does not matter how much you promote something no one wants.

I do not pay to promote. If I did it would be another fee to pass on to buyers. I'd rather do some extra work and keep the prices down.
You can work to promote listings yourself. Make sure you use every character available in the title to pull buyers in. Your books might benefit from including the ISBN # in the title. Make sure your pictures are top notch by showing different angles and closeups of some features. Research fair market values of items and price accordingly. Optimize shipping with lower costs by passing on discounts. Buyers will do basic math and your shipping price may make or break the deal. Have sales and offer lower prices on items you've had listed for a while.

I would never waste title space by including the ISBN when it's searchable if you enter it in the item specifics, btw. We sell almost exclusively books and have never once put the ISBN in the title field, I don't think.

That is not necessary. If you have it in the ISBs [item specific boxes] it will pull up if there is a Google search by the ISBN. I just tested it, so it still works.
I agree with @brightlightbookseller , it simply isn't necessary to waste space in the title.
