A buyer paid on paypal for item plus the shipping. Then he puts a note on it asking if he can pick it up, I would only do that if it was very near by to me, but.... I dont feel that this is always safe plus I was already charged shipping fee for the item on paypal which was $1.30. Should I just say no it is not convenient for me, or say I will refund your money on paypal

1 - Sure when would you like to come by?
2 - I'm sorry I'm not set up for pick-up, I'll be shipping your order today so you should receive it by Friday.
3 - Obviously you are a thief and scammer I am cancelling your order, refunding and blocking you!

But without online tracking, how would you prove buyer did indeed pick up the item.
If it were me, I would tell buyer that you are going to cancel using problem with buyer's address.
Then list as pick up only where he comes pays you in cash, and then leaves.

The odds one would ever need to prove the buyer actually picked up the item are slim and none.
We are probably talking about a $5 item with a seller investment of NOTHING other than 50 cents in eBay fees.

I would personally let the buyer come and pick up the item. Is there anyone else with you? If you're unsure, just tell the buyer no and ship the item instead.

For various reasons I no longer do local pick up.
The biggest reason being that there are just too many crazies in the world today.
Everyone has to do what they are comfortable with for their own business.
Why did the buyer pay for shipping anyway if they intended on picking up the item ?
They should have been asking you about this before the purchase- not after.

Since we have no way of knowing the OP's particular circumstances, we cannot say that your #1 would have been the best option or even a viable idea. Some of us live alone and are not too keen on a stranger arriving at our door to pick up a purchase.
Personally, you'd be doing #1. Personally, I'd be doing #2.

If electronic payment has already been made, you're already skrewed with local pick up. A trail has been left!
The whole point of local pick up is exchange of cash for item. No need to "mark item paid" or "mark item shipped". Thus, no trail. Buyer can't file INR because it appears item not paid for. Seller could file UPI, but what would be the point? ebay, thus, left out in the cold as far as fees as it would appear no transaction never took place.
Fees of secondary concern to sellers looking to do what they can to protect themselves cuz ebay likely won't without electronic footprints.

Although I rarely get local pick-up requests any more, I used to drop off an occasional order at the buyer's office on my way to or from work. Once, I simply left the packaged item at my PMB so the buyer could pick it up there. Then I made a partial refund via Paypal, less any eBay fees. (Paypal adjusts their percentage so that's not an issue.)
