Buyer in Ireland purchased item for $119 in July - I shipped to the address provided, but it was returned to me. I'm not refunding her the $15.50 shipping, so do I just issue her a refund for $119.00 and eBay refunds the VAT?

eBay takes care of the VAT. The rest is up to you to decide.
I would subtract shipping, then I would subtract fees from the $119 and refund that amount.

Technically, eBay does not require you to refund anything when a parcel is Undeliverable. This includes shipments that are refused.
Ethically is another question.
I would refund the original payment, minus shipping both ways and ALL my fees, including the fee on VAT/sales tax, and my costs for packaging, including my labour at my provincial minimum wage of 25cents a minute.

Yea, in that situation it's yours. Even domestic, if it comes back the sale is over. I would love that situation just to win over ebay, but I morally can't to that to an individual. I would try to complete the transaction if I were you in that situation.
