Custom Form for Multiple Purchase

I sold 25 cothing pieces to a Chinese buyer.  The custom form will only allow 5 items.  Does anyone know how I claim all 25 pieces?  The package will cost around $300 to ship, and I'm scared to death they will return it for not filling out the forms correctly.  Thanks in advance.

First item: lot of five blah, blah

Second item: Lot of five blah, blah

Etc.

Bag five items together for a total of five bags of five items.

That is all you need, you don't need to itemize unless they are radically different items.
