There is no difference. The point is that you package everything together to arrive at that cost or near it.

If you sell a buyer two different listings and the price you state for shipping is $3.50 on each listing, then the buyer will be paying you $7.00 for the two items for shipping. It is unlikely that the cost to ship these two items together would cost you that much. So you give the buyer a discount on the shipping.
When you figure what that is, keep in mind that you pay FVF on shipping, so don't discount it so much that it costs you money you aren't getting from the buyer.

Nobody pays the difference. That's the whole point of combined shipping.
