Anyone else having problems with their rate tables for shipping? Since yesterday I have seen the check box that says "Apply rate Tables" completely disappear when revising and listing new items using Ebay and not a third party listing tool. It's creating a MAJOR problem with my listings since I have created international rate tables and now I cannot apply them to my listings.

Check on the "customize" menu on top right of new SYI page to see if the option to display it at all got magically de-selected by eBay gremlins while you weren't looking.
which pops up the customize menu
I don't use rate tables, but when I enabled them via the menu above, the "shipping rate table" text appeared as below. I clicked the "create" link to see what happened next, but it failed in Firefox 31 that I was using (could be due to FF31 being too old or use of Noscript or other addons, but I suspect its because eBay is using the latest and greatest spinny popup **bleep** for that menu)
I fired up a bare, unrestricted Firefox 40 instance, logged in, and hit a revise on that same listing. When I clicked the "create" it took me to a new shiny page I haven't seen before.
It looks like eBay may have rolled out that location specific rate table feature talked about in the last seller update! (the one where you can charge different amounts to Michigan or it looks like even not ship to Alabama at all) http://pages.ebay.com/seller-center/seller-updates/2017spring/shipping-tools.html
I didn't create any rules. so don't know if the selection checkbox would appear on the SYI form if I had created some rules.
That may be why your checkbox is gone: you may be involved in this rollout and your existing rules have to be redone via the new form?
Did you try the "create" link?

Hi @productioncars - Have you tried what @berserkerplanet suggested? I'm tracking another issue relating to rate tables through the bulk edit tool but I want to make sure this one is escalated as well if what was recommended doesn't work. Keep me posted!
