Thanks for posting, I'm sorry to hear you're having trouble adding notes to your purchase history.
I've attempted to recreate this on my Win 10 machine in Chrome, Internet Explorer, Edge, and Firefox, and I was able to add notes successfully on each attempt. I would recommend trying to clear the Cache & Cookies in your browser, and if that doesn't work, try a different browser. If that still doesn't work, let me know the browser you are using, so I can look into it further.
