Basically, I was unable to return items for a refund because of being in the hospital for almost a month. The items where not as described and one was missing.

Tell the seller you are opening a return. You can still open a return after 30 days but the seller can deny the return. Explain the problems with the items and see if the seller responds.
If you don't get satisfaction from the seller, open a chargeback with your payment services, either PayPal or your Credit Card if you used one of those.

I messaged the seller and they are allowing me to return them. Do I still start a return for each of the listings? Just so it is "officially" a thing? It says I am outside the return window and not eligible for eBay's MBG. I know to send back with tracking.

Yes, open return case(s). Choose items were not as described as the return reason as you stated. Make it official. See where that takes you.

You can return the items since seller is allowing you to return them. You can have tracking on them and seller has them. Here's the part where I am not sure about. What if seller has items...window to return items is closed by eBay. Seller might change mind...keep items and not refund you.
I don't know if it is one seller or more. Although you can do a chargeback...doing too many chargebacks might not be wise at one time from many sellers. eBay doesn't like it and your banking institution won't like it. And it will take at least 2+ weeks to get such a refund.

If the seller accepts the return(s), the seller is obligated to follow thru with the returns.

I never had to go through one of these. So there is a button where "seller hits that the buyer is allowed to return" something out of the 30 day window through eBay returns ? Not just through an email. And hoping that is what this buyer did and not just send an email.
