How do I deal with an EXCHANGE of sizes? All I'm seeing is Refunding.
Does my customer have to return for a refund, then repurchase?

If the buyer has already started a return, that's the best way to do it.

To do an exchange would require your buyer to return the unwanted item, so I'd say 'Yes' to a Return request/repurchase.

With some items (not specifically yours), it may make sense for you to refund them fully and let them keep the item you sold/shipped. (if the shipping back and forth exceeds the worth of the product)
Either way you will need to set up another listing for them to purchase (after refunding) the replacement item. Some buyers may be ok with purchasing another item immediately and willing to wait for the refund of the other to be applied. (they want/need the item) It is ok to give them that option as well.
