Manage Store Categories: unable to "add category", button no longer working

I am having difficulty adding new categories for my store.  I am currently at 60 out of 300 categories.  Every time I try to add a new category, the button to do this is non-responsive.  

I have tested this on three different browsers; Chrome, Safari, Firefox and all browsers tested  on two different computers.

I would like to know how to fix this so I can continue to work.
