receipt for paid hearing aid

I am the person who bought this Sony hearing aid. This hearing aid is covered by my insurance. To be reimbursed I need a receipt with the item I purchased, the price I paid, the date and something indicating I paid it. I would appreciate your prompt response.

I was told ebay must furnish this.  How do I contact them

If you need a receipt for something you bought and paid for, it would be better to address your message to the seller, rather than to a Board where the seller probably will not see it.
