Antique Help - What is this???

Hi - first time in need of assistance. Any help with this would be great - I thought it was an old silver and pearl pen knife, but the "blades" appear to be made of baleen or bone.  Could it be a pipe cleaning tool of some sort?  I 'm stumped.  Found it in a box of old sterling jewelry, measures about 2 1/2" long, everything folds into the body.  DSCF7828.JPGDSCF7830.JPG

It looks like a lace tatting tool of some kind.

Some of these were toothpicks but yours seems a bit thick. I have a few  stanhopes (souvinir minipics) set into a pivoted bone toothpick like yours; the Stanhope on one end and the toothpick/point on the other end.  I have one that has a grid end for scoring leather used prior to sewing sturdy fabrics or leathers and a punch/point. There are probably a bunch of similar items listed on the ebays..
