If you have less than 300 items (including ended items) on your watch list you should be able to add items by clicking the "heart" once.
To create custom wish lists
On the website

Click Add to Custom Wishlist
or use the drop down at "My eBay Watchlist" and select "Create a new list"
On the app, use the "Add to Watchlist" on a listing
At the bottom you'll get a pop-up where you can add it to other lists.
Keep the main list not full, it makes it easier to manage. Sometimes I have to add it to the mainlist and switch to custom list from the page linked above.

It appears you have to save it to the watchlist before adding to the custom watchlist. Here's what's going on: I look at an item, and when I click the add to watchlist bubble, the bubble greys out and stays that way .No pop up to add to another watchlist. I'm on a new Windows 11 PC. Seems to have started with the new PC.

It used to allow you to pick which list to save to, right now it seems you have to save to the default list first.

The only time I have been unable to add is when I had 300, ended listings don't drop off.

I have 3 items on my watch list. It's just really weird. Ebay has been no help either.

I deleted the three items on the watchlist, and still can't add an item.

Try another browser to rule out a browser issue. Or try signing out of eBay completely and signing back in. Sometimes a device restart can be helpful, as deferred updates sometimes need to install.
Be sure you are looking at the entire Watch List, and not just a subset. Try this link:

eBay has raised the maximum number of Watch List items; I am not sure what the current maximum is, but at the moment I have 770 items on my Watch List, so it appears to be greater than that. Check to make sure you do not have a lot of old or ended items on the list, particularly if you are using the eBay app, which defaults to showing active listings only.
If you are using a desktop browser, you should be able to create custom lists from your Watch List page by clicking on the large "Watchlist" heading at the top of the page.
Be very careful moving items from your Watch List to other custom lists. At the moment, if you delete an item from your Watch List, that will also delete that item from any custom lists as well. This can make it very difficult to move items between lists if you cannot add items to your custom lists directly from the item listing.

I have no items in my watchlist. Signing out and signing back in does not solve the problem. This started with a new windows 11 PC I bought recently. Both my old PC and this one used/uses Firefox as the browser. As I said in previous posts: when I try to add and item to my watchlist ,the bubble turns from white to gray and does not save the item.

Great to hear the watchlist number is higher, I did not know that deleting from one deletes from all. I don't think it was always like that, but glad I know now.
@jm4438 Can you try another browser - like Chrome or Edge just to see what happens?

This is a relatively recent change in the last couple of years or so -- unintended, I think. The other custom lists operate differently; when you delete an item, you are given an option to either delete just from that list or from all lists.
Only the Watch List defaults to deleting from all lists without any option to stop that.
It appears that when the option to delete from all lists was added to the custom lists, the operation of the Watch List deletion was changed as well, but not for the better.
This has been reported to eBay in multiple ways, including by tagging a member of the community team, but so far no indication that eBay intends to correct this.

I just had a thought after adding something to my watch list.
If you are trying to watch something that has "variations", you have to chose the color/size/item before you can add it to your watch list.
example, I had to pick a size and color for this t-shirt before I could put it on my watch list. I couldn't just add the listing without those choices.
