"add to cart" button randomly missing from our listings

The "add to cart" button is missing from many of our listings - randomly.
We have over 5000 listings, and this problem appears to be random - some buyers can add to cart and buy multiple items at once to take advantage of combined/free shipping, while other buyers are forced to buy and pay for items one by one. This happens randomly for both U.S. and international buyers, so it is not site-specific.
All our items have been listed the same way, we offer combined shipping on all our items, we have not chosen "require immediate payment" on any item.
We have been selling on ebay since 1999, this problem arose about two months ago.
Any ideas?
