Why are items showing up in my cart I didn't put there

I keep getting notices that I have an item in my cart I DID NOT put there.  The last item was something I wouldn't even consider buying so why would I even be on that page to hit I wanted to buy it.  This happens ALL the time not just occasionally.   Does anyone have this happen too and how to stop it.
