What comprises the "Transaction defect rate"

I noticed that if Ebay reviewed my account today they would rate me Below Standard.  There are two matrix which are not normal, late shipment and the Transaction defect rate.  The late shipment is obvious but I cannot find, anywhere in the help sections, what comprises the Transaction Defect rate.
If it is what I think it is, how does one go about having the Ebay reviewers/powers to be, to  consider the causes which were both largely customer caused and limited ebay option caused?

What action do you take when someone doesn't buy the add ons?

If you are refunding or cancelling these sales, you are recieving defects for those transactions. That would be the reference to your transaction defect rate. Thats just a guess.
