Late Shipment Rate Error? What is going on?

So Ive been selling 150-170 items per mnth for the last 5 months. I have only had 2 total LSRs

All of sudden this month Ive gotten 28 Late shipment notices!!! Im at 6.24% on LSR but my Tracking Uploaded and Validated % is 96!

I havent done anything different. I ship same day!

Then when I click on reports to see which transacitons are getting dinged with LSR they all show Late Delivery. However when I click on ALL of them the delivery tracking shows them being well within the delivery range for Standard shipping.

What is causing this error? It makes no sense and its ruining my Top Rated discount
