How do you add a freight company for orders/items over 150 lbs.? and how do you apply it to the item your selling?

Generally with freight you wait and get a quote to the buyer's location after the item sells.  You then invoice the buyer for the item plus freight.

You can contact your local freight companies and get some sample quotes for a couple of locations, one near and one far, and give potential buyers an idea of the cost.
