How are product exchanges handled when a customer starts a return?

We've come across this problem a few times.  Customer requests a return but in the notes or through private message they request to have it exchanged.  However, Ebay considers this a return for refund and we are always under pressure to refund the order even though an exchange was done.

 

Any advise on how to handle exchanges?
