If you want to exclude items that have a certain keyword, put a minus sign in front of that keyword when searching.
Note that using an exclusion term has side effects. Normally a search will also match other keywords that are considered synonyms of your keywords; when using an exclusion term, only literal matches of your keywords will be returned.
Some other details of how searching works can be found in this old page on eBay Ireland:

You say you want to "exclude items". Besides the "-" sign to exclude keywords, you can often exclude multiple items that are not what you are looking for by using the filters in the left column (or in the popup from clicking "More refinements at the bottom of the left column): narrow down the category to get rid of items that match the keywords but not the category or subcategory (e.g. choose the accessories subcategory if that's what you are looking for or the subcategory for the main item if you don't want to see the accessories--though far too many sellers list the accessories in the main item subcategory); choose name brands; if you see sellers selling something else you can exclude them with the sellers filter by changing "Include" to "Exclude").

< put a minus sign in front of that keyword >
Make sure that the hyphen is actually touching the left side of the keyword.
