I got dinged for one that was sent out on time but my local post office
didn't get the acceptance scan and then the post office mis routed the
box and it got delivered late because of that. How exactly is mis

Yes this is how it works. You could sell 20 things all to the SAME buyer
at once and that buyer can choose to type out 20 feedback comments to
you but those 20 feedback comments will only count as one on your
feedback number.
They can leave you 14 feedback remarks but it will only count as one.
That is just how it is. If they come back at a later date (cant remember
how may days exactly) and make another purchase it will count again.
This is why a created a second selling ID where I wasn't worried about
trying to maintain the top rated seller status. I sold a bunch of window
clings and just shipped them out in regular envelopes with stamp
