“Destination Country Discrepancy”

Recently, I sent one registered item to Thailand and one to the UK. They came back to me with labels that said the following: “Destination Country Discrepancy” Your package was rejected for export as a discrepancy between the electronic customs manifest transmitted to USPS and the physical package attribute has been identified. The destination on customs declaration form must accurately represent the destination on the package.    

 

My local post office said that it was returned because the item needed a customs form. Anybody else have this issue? I
