When selling an item where the shipping cost is high, If the buyer claims a problem and eBay issues a refund do they refund the original shipping cost back to the buyer?   Additionally... Can the buyer refuse to return the item because the shipping cost is too high?

If the return is a seller failure which results in a Money Back Guarantee claim, the seller is responsible for two way shipping.

You (the seller) pays for the return shipping.

So the cost of return shipping is not a concern of the buyer.

If there is a problem with the order, you are responsible for the return shipping. Once they open an item not as described case, ebay generates the label for you and sends it to the buyer.

Exactly. That "ebay generates the label for you and sends it to the buyer" is the key. eBay makes it pretty easy for buyers to do returns no matter what, and usually at the seller's expense. There are some buyers who wouldn't care the least if return shipping costs the seller 50 bucks as long as they didn't have to pay for it, even if it's a remorse return, and then there are other buyers who are so thick that when it comes to the "free shipping" transactions they really believe that nobody has to pay for shipping costs.

If the buyer files a dispute "Not as described" or "Item not Received", the seller refunds the buyer's entire payment including shipping. And if the seller wants the item back, the seller must pay for the return shipping. 

 

If the buyer requests a remorse return, the seller refunds the buyer's entire payment including shipping. For a remorse return, the seller pays the return shipping only if he offers free returns.

To avoid problems with refunds and returns when items are large or heavy, you might want to consider local pickup or use CL or FBM.

If ebay issues a refund? It'll be a full refund guaranteed, including the original shipping. You NEVER want to have ebay issue the refund, that usually means you weren't able to work anything out with the buyer. What you want to do is proactively refund them yourself after the item arrives to you. That's when the clock starts for ebat telling you when you must manually refund the buyer by. Once that time passes and expires without a buyer refund, it's likely ebay will issue it for you. Depending on your seller level, you may be able to keep the original shipping cost when you manually refund. This also depends on what reason the buyer chose when they filed the return.

Yes buyers can refuse to return the item for any reason at all but they would not be protected from buyer protection if they refuse and ebay will not force you to refund them without having the item back to you.

It gets to be a on ongoing conundrum.   
I expect shipping to cost $xx, do I even list this item, or just throw it away?
And everybody that want's their item Expedited (next day or 2nd day air).  NO, too much liability, the risk of return is not worth the reward.
I very seldom sell Sets of things (such as wheels).  Wheels take two to four boxes, none of which is cheap to ship.  If there's an issue with one,  ALL four are coming back.
It's bad enough that I won't even list four matching wheels one at a time anymore.

I think OP is referring the original shipping cost. Like say they file a remorse return but decide to not send it back because the seller doesn't have free returns and the original shipping is high.

On remorse return I have the option to NOT refund the initial shipping cost when it is a separate amount that was paid by the buyer.

There isn't a consensus about this here. The reason is when a buyer files a remorse return and it arrives to the seller, the default option ebay gives for refunding is not including the original shipping charge. And when that type of refund is manually processed, the return gets completely closed versus staying in the returns page as a "Waiting for buyer to confirm refund received" when you issue a less than full refund.
