I created a Volume Pricing discount on some of my items, using the
seller hub, did NOT select the SINGLE ITEM ONLY option, created a rule
to apply the discount to every item in my store's APPAREL category. It's
not working. Did I do something wrong? ...

What kind of book was it? Not all books qualify for media mail. If it
has a lot of advertising in it, and the post office inspected it, that
might be why you had the additional charge.
