The only idea is to double check your combined shipping settings. I use combined shipping and it works as designed. I use flat rates, if it makes any difference.
I checked a few of your listings and don't see combined shipping on them. If you have set up combined shipping rules, have you also checked off on each listing (on the listing form) that you want to use that rule?
