You have a 45" RPM 7" Japanese original mono release from 1963, Michiyo Azusa – Kon'nichi Wa Akachan (translated to "Hello There Baby" (literal translation: Good Afternoon, Baby)). The Flip Side is Itsumo no Komichi De (Translated to "The Path You Always Take" (literal translation: Always By This Path (or narrow lane))

When looking up records, go with the model number on the lower left in picture #2. Glad you have the sleeve.
