I have a great buyer who has been very patient in waiting for the UPS claim on an item damaged in shipment. It has been approved. Will my managed payment fees be reimbursed when I refund? Does the buyer need to start a "return" of sorts first?

My only comment is the word patient. You should have refunded the buyer immediately, then filed your claim.
