A CD with no case can be shipped in a stiff cardboard mailer as non-machinable first class mail or in a box at a package rate.
A CD in a jewel case should be shipped in a box, since jewel cases crack easily.
A CD in a more flexible clam shell can be shipped in a bubble envelope.
Music CDs and movie DVDs can be sent via media mail. Software CDs are not on this list:

Depends on it's value. A box is always best but more expensive. Envelopes can resualt in breakage along with feedback from unhappy buyers. Netflix used to send them in just a paper envelope and MOST got there ok.
