I am confused about the right steps to do an exchange. Ebay help pages aren't providing guidance.
The situation: I sent a buyer the wrong item by mistake and have the right item in stock. I would like to do an even exchange at no cost to the buyer. They have not started a return request yet. My listing was "seller does not accept returns." What is the right way to do an exchange so I don't accidentally send a refund and I am able to generate a return label for the buyer at my cost?

Because eBay isn't set up for straight up exchanges. The buyer can start a return and you can accept, and pay for the label. Just follow the directions on the screen.

You might have to let them send it back at their own expense and then refund them the amount they paid for shipping.
Once you receive the item back, then ship them the correct item.
Most sellers offer 30 or 60 day returns. It's just good service, gives buyers peace of mind, and you will sell more because buyers will have more confidence.
