Hi,
A foregin bidder bid on an item that was not intended to be bid on by someone outside the US. I figured since I've not had a problem in the past with overseas shipments, I would just send it.
I got an email from the Italian postal service that I couldn't read because it was in Italian, so I just ignored it. Three weeks latter the guy says he never got his package, opened a case against me and now my funds are on hold.
I don't think USPS offers insurance for overseas shipments.
My questions is: am I responsible for refunding him without even getting the item returned first?

I don't think USPS offers insurance for overseas shipments.
For certain services they absolutely do. They don't for first class shipments but those can be insured through a 3rd party.
What was the item and the declared value?

You are required to issue a refund if you don't have proof of delivery unless you've got proof that the buyer refused the package. Google translate will help you translate the letter you received.

I think what the OP meant was if they needed to refund before the package was returned to them through post.

Yup. I'm thinking that was an official notification of a prohibited item that was advising it was confiscated or destroyed, or hopefully is being returned to the seller.
