My combined shipping is set up $12.99 flat rate + $1.00 for each additional item. This is for International:
Buyer is in Poland and when they go to check out a warning comes up that says "Seller does not offer combined shipping discounts..."
Not sure how to resolve this without having to charge them double shipping and immediately refund them (which I'm not sure they can do). What am I missing?

You're sure the combined is setup on both items? In the longer term, you might want to look at setting up business policies. It's a lot easier to manage a single policy that applies to all (or a specified group of) your listings.
You might also want to check your "shipping preferences" under your account settings. It's convoluted, but I have seen some odd behaviors like this arise when shipping preferences at the account level don't match what's set on the individual listing.
Also, as you adjust, you should be able to see the same policy/price that the customer sees by clicking on your listing, view details, click on the "shipping/returns/payments" tab, and then change the country to "Poland".

Did they say they were being charged 12.99 x 2? $14.99 should already be showing if they ordered 2 of the exact item. If they are 2 variations in the same listing or 2 different listing than you need to set up combined shipping rules in your shipping preferences in order for the discount to work. That screenshot that you are showing only works for the identical item, not a variation.
I don’t know why they are getting that message but check to see if you have checked off that you allow combined shipping in your preferences.
