A buyer made me an offer, and I responded with a counteroffer. Can my item sell to someone else during the hours the counteroffer is being considered? Or is my item "on hold" until the time runs out for the consideration of my counteroffer?

An offer is not a hold. Any number of offers and counteroffers can be active at the same time, and meanwhile the full "Buy it now" price is also still available. As soon as somebody accepts an offer or counteroffer, the item is considered Sold and other offers just expire.

Yes. Someone can click on buy it now and take the item from under you.
Some sellers do not respond to OK offers hoping that this will happen.

Offers are not binding. Anyone else can bid, buy or offer at the same time.
