I search "handmade Christmas ornaments" the only filter I have on is Item Location: US only, My results are 11 Hallmark leg lamp ornaments. I searched "James Patterson book" I get 2 results 1 is actually a James Patterson book and 1 is hollowed out book stash box thats not even a james patterson hollowed out book.
Those are very broad search terms, There should be thousands of results, what's going on?

I just did your first search.
"handmade Christmas ornaments" with US only.
I came back with 360,000+ results.
The search seems to be working fine for me.

Make sure you're searching all categories and not a specific one.

A US-only search for handmade Christmas ornaments:
A US-only search for James Patterson book:
https://www.ebay.com/sch/i.html?_nkw=James+Patterson+book&_sacat=0&rt=nc&_sop=10&LH_PrefLoc=1
If you do not see approximately 340,000 and 65,000 results respectively for those searches, check to make sure you have not inadvertently changed your location to calculate your shipping estimates to some obscure island that few sellers ship to.

Search results are always dependent on the parameters used. As an example my numbers differ from yours as well as eburtonlab.
I did a search for "handmade Christmas Ornaments", exact words, exact order and US only and got 1,400 results.
The same search using Handmade Christmas ornaments, exact words any order, US Only yields 160,000 results.
Searching for James Patterson Book, exact words exact order, US only I get 378 results.

So everyone is getting a different amount of results
They are different searches.

Potentially. As I noted in my examples the search results are the output of the parameters you use for the search. This is especially relevant when it comes to which of the 4 selections you choose in the keyword options drop down menu.
