How do you combine shipping when selling more than one of the same item?

If it is of the same item to the same buyer than they likely paid for them both at the same time.  So there should be no issue.  Just package them together and create the shipping label from the payment.

When you have a multiple listing, If you are using flat rate shipping, there should be two spots for entering the shipping cost, one for the first item and one cost for any remaining items.

Are you talking about the clock cover?

You have the listing set up for:

$14 for the first item, $14 for each additional item.

If the customer bought 2 and you would like to pack them in the same box that's fine.

You can give them a discount if you want  but you are not required to in this case.
