A buyer made a purchase & paid. Before I even shipped the item, the buyer requested a return. After sharing messages, we agreed to cancel the sale. The buyer cancelled the request for a return. However, the request still blocks a cancelation. My question is about giving a refund without the option to cancel. Do I still get my fees back? I know I would if I had the option to cancel on the buyer's request. What about if I just give a straight refund without eBay knowing why? I do not want to be stuck with the fees when the buyer backs out.

FVF fees are refunded when you use the "send refund" option. Not sure on PL fees, but I doubt it. No refund on donation percentage. But that's probably the same for "cancel order".
There's really no other option for you though. If any other fee is not refunded I guess you can talk to ebay and see what they can do. But it won't be automatically refunded.

Wow! Just wonderful. I go to send a refund but now the buyer asks eBay to step in. Of course, they will close it in his favor & I will get stuck with the fees plus have a strike against me! People that do not know how to work through the channels really annoys!

If they closed the return, how did they ask Ebay to step in?

It would be very, very easy for eBay to add some computer code that would take note of your message exchange with the buyer, make note of buyer's cancellation of the return request, and then conduct a rudimentary dialogue with the buyer to steer him clear of opening a case so that YOU, the seller, would not be penalized.
Why eBay does not do this is absolutely mystifying to me.

No idea! lol You'll have to ask eBay that question. I think seller protection worked out this time. The buyer was refunded & there is no impact on my seller performance. The message said I do not need to reimburse the buyer or eBay.

You should have just refunded the buyer when the Request was first opened, the buyer would get their money back and eBay would have used the fees paid to partially cover the refund.
As @kensgiftshop asked, how could the buyer escalate a request that had been closed?
