How would this work? I have a local customer asking for local pick up. The item is rather heavy and would cost about $20 to ship. How can I do this without getting in trouble? Can I just change the listing to local pickup?
I would have to put an address in the message center, and it would not be my address. I will have to meet them somewhere, not letting them come to my house.

Yes, change the shipping to local pickup. Remove "Immediate payment required" if it's on the listing, since you'll be asking the buyer to pay in cash. After he purchases, you will be able to communicate more freely.
Around here, many police departments have set up an area in their parking lot that has surveillance cameras that help ensure a smooth process. Also eliminates the need to exchange any personal information about location or contact info. Even if your local office hasn't set this up, it's a logical place to meet anyway.

I was confused by the buyer will pay cash. I was under the understanding that the buyer had to buy through ebay, even though it would be local pick up. Can someone please explain this a little more? How would ebay get their fee?

The buyer purchases the item through your listing, but doesn't pay online.
eBay charges their final value fee when the item is sold.
