Sold my first item for local pickup. Buyer accepted the offer and I sent them an invoice with Pay at Pickup. Do they get an email with the QR code and just bring that when they come for pickup?

Yes. It helps to also exchange feedback right then and there.
Mark it paid, Shipped, Feedback left, Feedback received and you are good.

Why would you wait for payment until the moment of pickup? I would never agree to that. My customers pay first before shipping, which is the same as getting in my truck to meet somewhere. I allow three days for me to deliver, courier the item or to meet after payment.

eBay has always strongly recommended that items sold with local pickup be paid for in cash by the buyer when he picks up the item from the seller. If the ubuyer pays with Paypal before picking up, the seller has no proof of delivery in case of a dispute. Of course, now that the buyer has a QR code that the seller can enter into the tranaction to prove delivery to the buyer.
Yes, if something is being delivered, assuming you mean shipped, of course the buyer would pay before the seller ships.
